Outstanding Member Nomination Form

Submit by 5:00 pm Wednesday, October 11, 2023.

An outstanding member must have 4-H related achievements that measure above and beyond other members.  Please provide 4 Reason Specific Examples why this member is outstanding in the 4-H Award area. Be specific, creative and detailed in the outstanding accomplishments of the member.  A member must have a gold seal record book to compete for an award.

Anyone can nominate a member. Members can nominate themselves.  Sample areas of nomination: any 4-H project area, achievement, attitude, willingness to help, willingness to be involved, citizenship, community service, cooperation, team player, demonstration, leadership, club officer, public speaking, design your own 4-H related award area.
